The Toras Chaim (160a DH Mekushar)asks this question on Rashbam. He does not give an answer.
Perhaps we could answer that Tosfos is suggesting that it may be forgotten over time that the Shtar was Mekushar, and as such we may one day not take notice of folds and holes, and would wrongly come to assume that it was a Pashut.
I am not aware of a Gemara that deals with a Shtar and discusses whether it was a Pashut or Mekushar.
