Is the Tarah Kama requires both three full lugim of Shoavim and a Mareh of mayim (The implication of milk case is that had there been three lugim and maintained appearance of water-it would pasul) is needed to pasul a mikveh pachos 40 seah.
Rav Yochanan Ben Nuri is machmir Three lugim of anything always go after the color so case two where the kortov of milk completes the three lugim and maintains the appearance of water it is Pasul.
Rav Kahana's question in terms of Shitas Rav Yose with mei Tzeva might be the shita of Rav Yochanan Ben Nuri (possibly because it's label is dyed water-stems from it appearing mostly like water).
Our Gemara adds Chaser to case one (possibly for consistency in the wine and milk case). But by doing so it inverts Amar Rav Yehudah amar Rav, and it explains Rav Chiyah taking it down. HOWEVER per the Mishna in Mikvaos I don't know who Rav Chiyah would be going by, unless he observed that in his case of 3 lugim the mikveh water there prior changed color. He did not take it down because of the Tana Kama.

I'm not sure I understand your questions, so here is a summary:
Rav Yehudah Amar Rav: 3 full Log water plus wine and looks like wine: the Mikveh is valid. Rebbi Chiya: the Mikveh is invalid.
Mishnah in Mikva'os: There are 2 versions in the Tana Kama:
(1) "Less than 3 Log of water" plus wine and looks like wine: the Mikveh is valid (but 3 full Log of water would be invalid). Rebbi Yochanan ben Nuri: Even 3 full Log is valid.
(2) 3 [full) Log of water is valid even according to the Tana Kama. Rebbi Yochanan agrees. He argues in the case of milk, that less than 3 Log of water plus milk is invalid.
Rava goes with the first way, so Rav is only like Rebbi Yochanan.
Rav Papa is in doubt about the Girsa.
Rav Yosef and Abaye go with the second way, and therefore Rav is also like the Tana Kama.
The question of Mei Tzeva is a different point; water with paint is always water, but water with wine can be wine.
