Chasam sofer in his teshuvah YD154 explains that Levi holds that even though the blood comes from the same place still the causes are different. In others there is 1 maayon but 2 physiological reasons. Rav disagrees with Levi then must hold that causes are immaterial. As long as comes from the same place it is dam tomeh. To say, that Ravi argues that there is one cause would be a machalokes in metzius which is not satisfying.

Since we pasken like Rav, how can we reconcile the many acharonim who define dam makaoh in the mekor as being another cause. For example, we find that the Chazon Ish and many current poskim calling a biopsy of the mekor as a makoh. This has always troubled me.

The answer is that surely we differentiate between Dam which flows from the Makor, and Dam

Makah. Dam Makah is not merely a different cause, but it is a different phenomenon altogether.
