The fifth additional bracha is "shomea tefilla". But that is the same as the bracha is "shema koleinu". Shouldn't each bracha be different?

Actually, only the finish of the Berachah is the same. However, you do make a good point -- it is certainly unusual, but I did not find any commentary on it, and it does not have to be called problematic.
